Issue:
Information was accidentally faxed to the wrong fax number. The recipient of the information contacted the Release of Information (ROI) Section to report receipt of the information. The Privacy Officer called the individual who received the information by mistake and asked that the information be shredded. This was done and there is no indication that Veteran A's protected health information was disclosed to another third party. The information at risk included Veteran A's name, full SSN, date of birth and diagnosis. Update: 03/14/12:Veteran A will be sent a letter offering credit protection services.
Outcome:
Privacy Officer will do a second check of all forms faxed to Community Agencies to make sure that phone and fax numbers are correct.
