Issue:
A VA employee reported a privacy incident regarding a patient receiving a MyHealthe Vet in-person proofing form to process by a clinic team. The patient was given the paperwork on another Veteran. The paper work included the Veterans full name, last four of social security number, appointment information and lab/medical information written on the back of the sheet. The Veteran told the clerk at the check-in the paperwork was not his, the clerk told him it was ok and to take it to the release of information office to complete the process. The information never left the VA and the paperwork was given to a VA employee to safegaurd. Update: 03/28/12:Veteran B will be sent a notification letter.
Outcome:
Email sent to the supervisor to educate staff on incident.
