Issue:
Today, 10/7/2013, Chief reports that a Veterans financial application for Arizona military family relief fund financial assistance form was faxed to an unknown recipient on 10/4/2013. The fax number for Arizona Department of Veterans Services was obtained from a colleague who performed same functions previously; no phone number was provided. The contact at the agency notified the sender today that the fax was still not received. Upon checking, the last four digits were not correctly sequenced but were correct digits. The fax coversheet was used with the required privacy disclaimer language noting how to return unintended documents if faxed in error. No contact of a mis-directed fax has been made to the department or Privacy Officer. The documents included Veteran full name, full SSN, home address, telephone numbers, income, service related disability, medical diagnoses, DD214, and utility bills. Requested a fax machine report to address whether 13 pages were sent or not. Additional notification and investigation, pending.

10/07/13: Veteran A will be sent a letter offering credit protection services.
