Issue:
An employee's backpack was stolen from his car parked near his residence. The backpack contained personal belongings, Personal Identity Verification (PIV) card, keys to his VA office and a caseload list containing 31 Veterans' last name and last 4 of SSN. No other personal information of Veterans was on the list.
Outcome:
11/13/14: The Incident Resolution Service Team has determined that there was a lost Personal Identity Verification (PIV) card. This is a policy violation and no data breach has occurred. Additionally, a notification letter will be sent to the 31 Veterans.
