Issue:
Veteran A reported a delivery by UPS of two coolers of medication. Both coolers had a label with his name and address on them, however only one of the coolers contained his medication. The other cooler contained medication for Veteran B. After review, the Dallas Consolidated mail Outpatient Pharmacy CMOP determined that both Veterans orders were in the processing stage at the same time. During the final stage of printing UPS address labels, the technician printed Veteran A address label twice and never printed a label for Veteran B. The same address label was placed on Veteran A and Veteran B coolers. Veteran B's name and type of medication was compromised. Veteran A returned the incorrect package to his medical facility. The technician placed on probation and retrained on proper procedures. Update: 12/28/12:Veteran B will be sent a notification letter.NOTE: There were a total of 2 Mis-Mailed CMOP incidents out of # total packages (# total prescriptions) mailed out for this reporting period. Because of repetition, the other 1 is not included in this report, but is included in the "Mis-Mailed CMOP Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter
Outcome:
The technician at fault was placed on probation and retrained on proper procedures.
