Issue:

The Patient Advocate received a call from a Veteran who stated that he stopped at the front desk yesterday to ask how to get to Ortho and asked for a print out of his appointments. The volunteer printed out a piece of paper and gave it to the patient. The patient put it in his pocket. When he got home and opened the paper, on the paper was his name, full SSN, day of birth, and his Service Connection Rating. There were also the names of three other Veterans including name, full SSN, birthday and their Service Connection Rating. The Veteran has been contacted and will return the document to the facility. Update: 04/17/13:Three (3) Veterans will be sent letters offering credit protection services due to full name and full SSN being disclosed.04/22/13:Two (2) do not have addresses on file, so only one letter is able to be sent.

Outcome:

Volunteer had to re take privacy awareness training and was educated on how to properly use menu options on VISTA.
