Issue:
A stack of pamphlets was taken to the VA Medical Center's library to be folded by the machine. In the midst of this stack was a patient's medication list. A compensated work therapy (CWT) patient found it and brought the document to the staff in the library and they reported the incident to the Privacy Officer. Update: 02/07/12:Due to full SSN, full name, and medical information being exposed, Veteran A will receive a letter offering credit protection services.

Employees reminded to be more cautious when handling documents with PII.
