Issue:

Based on interview and record review, the facility failed to prevent unauthorized disclosure of protected health information (PHI) for one patient (Patient 1) when a nurse gave a sticker with her information on it to the daughter of another patient (Patient 2). This failed practice resulted in the potential for physical, emotional, and financial harm to Patient 1.Findings:During an interview with the facility Privacy Officer (PO) on June 24, 2014, at 3:45 p.m., the PO stated on June 16, 2014, a nurse printed out a sticker (designed to be placed in a medical record for patient identification) for Patient 1, and gave it to the daughter of Patient 2, who had requested one with her mother's information on it.The patient sticker that was given to the daughter of Patient 2 (with Patient 1's information on it) was reviewed on June 24, 2014. The sticker included the following PHI:1. Name (first, middle initial, and last);2. Date of birth;3. Sex;4. Medical record number;5. Account number:6. Physician's name;7. Admission date; and,8. Location in the hospital. Failure of the nurse to verify the correct information on the sticker she gave to the daughter of Patient 2 resulted in disclosure of medical information belonging to Patient 1 to persons not authorized to receive it.
