Physician gave 8 letters to admin person to mail, the employee not reviewing the documents thought it was only to be sent to the cover letter address, so she sent all letters to one Veteran. Update: 04/12/12:Seven Veterans will be sent a letter of notification.
Outcome:
Employee was re-educated of safeguarding PHI and ensuring to double check all documents prior to mailing.
