Issue:

VA Patient A received his lab results and the lab results on three other patients. Patient A contacted VA about the Release of Information (ROI) error and shredded the lab results on the three other patients. This was reported by the Community Based Outpatient Clinic (CBOC) Administrator. The lab results did not contain any 7332 information but did contain the three patients' name, address, full SSN and protected health information. Update: 02/22/12:Three patients will be sent letters offering credit protection services.NOTE: There were a total of 20 Mis-Mailed incidents this reporting period. Because of repetition, the other 19 are not included in this report, but are included in the "Mis-Mailed Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

Credit Monitoring letters mailed on 3-7-12, redacted copy uploaded to PVTS. Consider case closed. Supervisor will monitor outgoing mail.
