Report ID: SPE000000063767, U.S. Department of Veterans Affairs

Reported Entity: VISN 11 Fort Wayne, IN

Issue:

A Volunteer accessed his sister's CPRS record. Update: 06/20/11: A volunteer through the VBA nonpaid work experience program accessed his sister's medical record. The Privacy Officer (PO) interviewed the volunteer. He had taken his privacy and security training and knew he was not to access this record. He stated he was curious and wanted to validate why his sister wanted to extend her stay here. The PO spoke with the volunteer's supervisor and to the employment specialist at the VBA. 06/21/11: This incident was discussed by the DBCT and it was the consensus that the Veteran should receive a letter offering credit protection services because the full SSN and protected health information (PHI) was seen by an unauthorized person.

Outcome:

The vocational rehab volunteer was terminated, and the Veteran was sent a letter offering credit protection services.
