Issue:
Veteran A contacted Outpatient Pharmacy (Nashville Campus) to report he received controlled medication, via UPS, which belong to Veteran B. Upon investigation, the Supervisor, Outpatient Pharmacy (Nashville Campus) , found the tech (who is a new employee) printed the wrong UPS label and the pharmacist, who checked the order, did not catch the error. Arrangements were made for Veteran A to return the mis-mailed medication to Outpaient Pharmacy via UPS envelope. The Provider wrote a new prescription for Veteran B's medication.Veteran B's information includes: full name, address, and an information sheet on the controlled medication. Update: 08/23/12:Veteran B will be sent a letter of notification.
Outcome:
9/5/12 - Upon inquiry, the PO learned: The Pharmacy Tech printed the wrong UPS label. The Pharmacist, who checked the order, did not catch the error. This is the first incident for both employees. Employees were educated on the importance of checking medication for accuracy before mailing. Additionally, arrangements were made for the other Veteran to return the mis-mailed medication to Outpatient Pharmacy via a UPS envelope. HIPAA notification letter mailed.
