Issue:

A clerk reported to the Privacy Officer (PO) they found a document which they turned into the PO. The document contained two Veterans' information that were admitted on a hospital floor. The information on the document seemed to be a list of information for the provider to use to treat the Veteran. The information contained included: the Veterans' full name, last 4 of SSN, Allergies, Code Status, Medications, Admit Diagnosis (including one Veteran that had 7332 information noted), Problems, extra information to know about the Veterans including follow up and actions to do. Update: 0217/12:Two Veterans will be sent a letter of notification.

Outcome:

Residency Program Coordinator to educate resident and attending regarding incident and for safeguarding of information. Notification letters to be sent to two Veterans.
