Issue:
It was reported by the Chief Business Officer that the VA is unable to locate 51 authorizations from November. A search is being conducted.
Outcome:
04/03/15: The Release of Information Office received the authorizations. They were logged into the system for processing, but they are unable to find them at this time. The Chief, Business Office Service believes they may be misfiled or in a stack of documents for scanning. Staff members are searching for them. 04/14/15: They were ROI authorizations, which typically have full SSN included on them. Two of the authorizations have been found. The service has asked to be given until 04/24/15 to find the authorizations. 04/15/15: Credit protection services will be offered to all 49 Veterans whose information has not been found. If any of those authorizations are found prior to 04/24/15, credit protection services will not need to be offered to those individuals. 04/27/15: Seven authorizations were found, so the total number of affected individuals is now 42.
