Based on record review, the facility failed to protect the private health information (PHI) of Patient A. This failure caused a breach of Patient A's PHI.Findings:The facility Breach Report, dated 7/19/13, was reviewed on 7/22/13. The report indicated Admitting Clerk 1 gave a patient a Conditions of Admissions form to sign that was labeled with Patient A's name and information. The information on the label contained the following: Patient A's name, date of birth, age, sex, medical record number, and account number. The facility investigated the breach, and provided training and counseling to Admitting Clerk A.
