Issue:
An address label and a Personally Identifiable Information (PII) label for Veteran A were switched and the PII label was attached to the outside of the envelop and sent to the Post Office for mailing. The PII label contained the name and full SSN of Veteran A. Update: 05/17/11:Veteran A will be sent a letter offering credit protection services, due to full name and full SSN being exposed.
Outcome:
Reeducation was done with staff involved. Reminded them of the importance of patient privacy and to be careful when mailing information to patients. Double check the make sure the label is correct and the information inside the envelope is for the correct patient. I also covered the process of what happens when we have a privacy incident. We offer credit monitoring to the person and how this effects them and also the VA.
