Issue:
Around 01/30/15 Veteran A, a Home Base Primary Care (HBPC) Patient, received the following medication belonging to Veteran B: Combivent, Symbicort, and Diltiazem. Along with the medication, Veteran A also received inserts, and possibly a Prescription Profile belonging to Veteran B. Prescription Profile printout containsthe prescription number, medication name and strength, and what condition the medication is for (i.e. "Take one tablet a day for Psychosis."). Prescription Label and insert contains prescribing information as listed above, prescribing physician and pharmacist name, as well as Veteran's full name (first, middle initial and last) and mailing address. No SSN or phone number is listed on inserts, labels or Prescription Profile printout. On 02/19/15 Pharmacy Supervisor was alerted to medication error by HBPC Pharmacist who received notification from HBPC Nurse. According to the HBPC Nurse, the medication error was discovered during her review of medication with Veteran A at his home. Medication was recovered; however, no inserts or printout recovered. Incident was reported to Privacy Officer on 02/27/15 by Patient Safety Officer.
Outcome:
03/02/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
