Issue:

Veteran A received medical records from Manchester VAMC Fee Basis Department from Employee A. Veteran A also received Veteran B's medical records. The records contained the last 4 of the SSN, full name, medical diagnosis and treatment. Veteran A received these records on July 3, 2012 and was returned to our Manchester VAMC on July 6,2012. The incident wasn't reported to us until July 6, 2012. The Acting Privacy Officer was notified and the Privacy Officer called Veteran A to confirm details. The Privacy Officer instructed Veteran A to return records to the Tilton Community Based Outpatient Clinic. Employee A asked Employee B to forward to the documents to the Manchester VAMC. The documents are now in VA control. Update: 07/06/12:Veteran B will be sent a notification letter.

Outcome:

The employee has been educated on the importance of protecting patient information and has been asked to retake the Privacy Training.
