Lab specimens were disposed of because the sample was exposed for too long. Update: 05/31/11:The patient will be sent a letter offering credit protection services, due to full name and full SSN being exposed.

Outcome:

The Acting Chief of the Laboratory Service has reeducated the all staff members on the closing procedures for the Laboratory Service. In addition, during the monthly staff meetings, the acting chief will be going over the closing procedures to ensure that the proper education has been disseminated to all Laboratory Service team members. Moreover, the closing supervisor will double check to verify all doors to the Laboratory Service areas closed and locked.
