Issue:

Veteran A's caregiver contacted the Dorn VA Medical Center Narcotic Pharmacist. The caregiver informed the Narcotic Pharmacist that UPS could not locate the medication mailed to the Veteran on 12/9/11. According to the caregiver, UPS attempted one delivery and, apparently, the medication was lost or stolen by UPS after the initial delivery attempt. The Narcotic Pharmacist verified with the DDS2 Customer Support Team that the last scan on the package was 12/13/2011. A tracking number has been initiated by UPS for this package. The Narcotic Pharmacist has reprocessed the medication, and the Veterans caregiver will pick it up at the Dorn VA Medical Center Pharmacy window. The information disclosed: full name, address, and the name of the medication and provider. The medication was mailed on 12/9/2011. Update: 12/20/11:Veteran A will be sent a notification letter due to PHI being exposed.
