Issue:

Today, 03/06/15, the Privacy Officer (PO) received several complaints that another staff member has accessed sensitive information of two (2) fellow employees. The staff member is alleged to make statements with accuracy about military service level connection and medical diagnoses. He claims his girlfriend can pull DD214s in her work in another VA agency. He claims that he knows other people within VHA who will spy in others information and then share with him. He complains to other staff that his service ratings are not as high as the other staff members. An Administrative Sensitive Patient Access Reports (SPAR) of all employees will be requested under this privacy complaint. Reports are concerned of his access into their charts. Additional notification and investigation, to ensue.

Outcome:

05/12/15: The Incident Resolution Service Team has determined that Employee B and C will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
