Issue:

Today, 07/24/14, The Release Of Information (ROI) Technician reports to Privacy Officer (PO) that Veteran B requested his colonoscopy report of 03/03/11. Later, Veteran B realized that this report contained Veteran As full name, DOB, full SSN, and full study findings but in Veteran Bs. Report was returned to the ROI desk, to exchange for his. However, Veteran Bs report could not be located. The Health Information Management Service (HIMS) Chief notified who will investigate with documentation department, address coding or data quality issues. PO and HIMS Tech will contact Veteran B when report is located to pick up. Additional notification and investigation, to ensue.

Outcome:

07/24/14: The Incident Resolution Service Team has determined that Veteran A and B will be sent a letter offering credit protection services.
