Issue:

On 02/21/14 a Veteran contacted the Privacy Officer (PO) inquiring about lost CDs. The PO spoke to the Veteran and was informed he never received his lost CDs. PO misunderstood when Veteran informed him he received his records in December 2013. The records the Veteran received were in paper format not CDs.

Outcome:

02/27/14: The Veteran stated he gave his Primary Care Team 2 CDs along with paper records from an outside provider. VHA Long Beach Primary Care Team never denied the CDs were in the envelope with the paper records, however they never physically saw the CDs. The CDs are lost, but the PO doesn't know if they were lost at the facility, or if the Veteran thought they were in the envelope along with the paper records when he submitted the envelope to the Primary Care Team. The PO was unable to determine the content of the CDs, not knowing if his full or partial SSN or what diagnosis were on the CD. The Primary Care Team didn't see the CDs so they weren't able to provide the PO with the contents. 03/03/14: Since it is unknown what is on the missing CD's at this time and to err on side of caution. The Incident Resolution Team has determined that Veteran A will be sent a letter offering credit protection services.
