Issue:

On 3/11/2011, Veteran A (father) received a reminder by mail of an upcoming Surgical Optometry Greenville Outpatient Clinic appointment. Veteran A was surprised to see that an additional appointment letter, which was intended for Veteran B (his son), included in the same envelope. On the morning of 3/11/2011, Veteran B (son) contacted the Health Administrative Specialist (HAS) working with the Greenville Community Based Outpatient Clinic (CBOC) by phone to report the incident. The information disclosed: Full name, last four digits of the social security number, address, and the name of the clinic. When: Veteran #1 received the envelope by mail on 3/11/2011. Update: 03/14/11:Veteran #2 will receive a notification letter.

Outcome:

Employees are being re-educated on the need to safeguard patient information.
