The above regulation was NOT MET as evidenced by:Based on facility investigatory documentation, staff interview, and medical record review, the hospital failed to ensure nursing notes included a record of pertinent observations as well as incidents and unusual occurrences and relevant nursing interpretation of such observations. In a written statement completed on 1/26/12, and during a telephone interview on 2/29/12 at 0955 hours, RN 1 confirmed she overheard SW 1 telling Patient 1's friend that her "baby might be taken away." RN 1 also stated Patient 1's friend disclosed to RN 1, that SW 1 had told her (Patient 1's friend) Patient 1 was doing drugs and the baby might be taken away. In the written statement, RN 1 stated Patient 1 was "so agitated" and Patient 1's friend was also upset.Medical record review failed to show nursing documentation describing this incident and failed to show an assessment of the patient or reflect any relevant nursing interpretations of Patient 1's condition and/or response to the incident.
