Issue:
Veteran A presented to Medical Administration Service (MAS) Staff to check in for a procedure, VA employee A from MAS placed the wrong Patient ID Band on Veteran A that contained another Veteran's Full name, SSN and DOB. Veteran A went upstairs to clinic and informed Employee B, she in turn directed Veteran A to return downstairs to Employee A in MAS to have the correct Patient ID Band placed on his wrist and remove the incorrect band. The Information never left the VA facility. Update: 12/10/12:Veteran B will be sent a letter offering credit protection services.NOTE: There were a total of 28 Mis-Handling incidents this reporting period. Because of repetition, the other 27 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
Outcome:
Credit monitoring Letter send out. The Clinic AO will be conducting training for all staff on the importance of reviewing all documentation prior to handling it to a Veteran, stressing the importance of safe guarding Veteran information.
