Issue:
Patient seen in SPU at which time they signed the imed consent form for another patient. patient then proceeded to the MSA Clerk for travel. At that time the Clerk identified that there were concerns such as there were no orders in the computer. the MSA contacted the PA regarding the orders and the PA indicated they were. This then caused the MSA to investigate further at which time she identified inconsistent signatures. ths then led the MSA to identify that the veteran signed an imed consent on another veteran. Veteran was also given a copy of the consent; however the MSA clerk did gain back the consent paperwork. Update: 11/02/12:One patient will be sent a letter offering credit protection services.
Outcome:
Staff education was conducted.
