Issue:
Based on staff interview, clinical and administrative document review, the facility failed to keep Protected Health Information (PHI) confidential when Patient 1's prescription form was given to the family of Patient 2.This failure placed Patient 1's PHI at a potential risk for unauthorized use.Findings:On 7/25/14 at 8:59 a.m., during an interview, the Accreditation Coordinator (AC), stated Registered Nurse (RN) 1 gave Patient 1's prescription form to a family member of Patient 2. The AC stated the RN failed to verify the name on the prescription form against the name band of Patient 2, per hospital policy.The PHI disclosed included Patient 1's name, date of birth, address, medical record number, phone number, diagnosis, and prescribed medication.The hospital policy and procedure titled, "Confidentiality" dated 8/2011, indicated "Policy. It is the policy of [hospital]to respect and protect the privacy rights of patients, their families, employees and third parties. All information that is deemed confidential by [hospital]...shall be kept confidential....".
