Issue:

A request for a Sensitive Patient Access Report (SPAR) was received by the Privacy Officer (PO) on 02/03/12. It was printed on 02/04/12 and the PO notified the Veteran/employee of the print completion. The report was picked up by Veteran/employee, reviewed and returned on 02/08 12, with list of alleged violations. The PO was asked to investigate the alleged violations. The investigation has been completed, a copy was given to the Veteran/employee on 03/19/12, and a copy was forwarded to Human Resoources (HR) for review, evaluation and processing. Update: 05/03/12:Due to fact that medical record to include full SSN and DOB was accessed inappropriately Veteran/Employee A will be sent a letter offering credit protection services.
