Based on interview and record review, the hospital failed to ensure that Patient 1's protected health information was kept confidential from anyone not authorize access.Findings:An interview with the hospital's Director of Quality (DOQ) was conducted on 10/4/12 at 4:05 P.M. The DOQ stated that Patient 1's prescription label was inadvertently placed inside Patient 2's prescription bag. Patient 2's prescription label was also inside Patient 2's prescription bag. Patient 1's presription label indicated the patient's name, date of birth, home address, and medication.
