Issue:

The Supervising Nurse emailed the Privacy Officer to notify that she was calling Friday's procedure patients and discovered the discharge instructions for Patient B were mistakenly given to Patient A. The Privacy Officer sent a message asking if anyone called Patient A to ask for the information back so we could correctly dispose of it and if counseling had been provided to the employee who made the incorrect disclosure. Update: 12/06/11:Patient B will be sent a letter of notification.

Outcome:

The supervisor has discussed this with staff. Employees will double check individual information against the medical record before releasing the information.
