Issue:
A Provider notified the Privacy Officer (PO) that she received a view alert. In an attempt to process it, she realized that it was her own medical record. The Provider then requested a Sensitive Patient Access Report (SPAR) and requested that the PO obtain justification for the accesses of four staff who showed up on the report. Update: 10/31/11:Provider/Employee A will be sent a letter offering credit protection services.
Outcome:
One of the Medical Administrative employees was interviewed. The Supervisor and the Service Chief reported to the PO that the employee questioned stated that he must have accidentally exited a patient's record prematurely. He then entered a note requesting a medication refill in the provider's medical record, instead of identifying her as a co-signer. He said that he thought that he was still in the patient's medical record. When the employee was contacted and questioned by the Privacy Officer, he realized his mistake. The PO removed the view alert and requested that the employee enter an addendum to the note. The PO retracted the note.Appropriate disciplinary action taken by management. Letter offering credit monitoring (signed by the Director) was mailed to the employee on 11/15/11. Redacted copy attached to closed this ticket.
