Issue:

Veteran A states that patient advocate mailed him some information. He received 4 pages of Veteran B's information. Veteran A does not have an appointment scheduled until 03/12/13. Veteran A will bring the information to the VA with him on that day and deliver it to the Privacy Officer (PO). The Veteran did not feel comfortable mailing the information to the VA. He states it is locked in his brief case and will remain there until his appointment on 03/12/13. He states he will not misuse the information but will keep it safe. Veteran B's name, address, full SSN and protected health information (PHI) was disclosed. Update: 02/28/13:Veteran B will be sent a letter offering credit protection services.

Outcome:

Supervisor met with employee and discussed privacy procedures and mailing requirements. A letter of counseling was provided.
