Based on staff interview and record review, the facility failed to maintain the privacy and confidentiality for one patient (Patient A), when an employee inadvertently sent Patient A's medical information to the wrong insurance company.Findings: An interview was conducted on 6/20/12 at 11:00 AM with the facility's Privacy Officer (PO). The PO stated that on 6/11/12 she became aware of a breach incident. The PO stated that an employee inadvertently sent Patient A's medical information to the wrong insurance company. The PO stated that the employee accidentally sent Patient A's father's information who has the same name.A review on 6/20/12 at 11:15 AM, revealed the following personal health information (PHI) disclosed in error included:Patient nameDate of BirthAddressPhysician'Medical historyThe PO further stated that the employee was disciplined and Patient A was notified of the breach occurrence.
