Issue:
A Canteen worker found a vehicle decal request unattended in the staff dining room in the cafeteria and handed it to VA police. VA police queried the employee who was working with the forms. She stated that she was never in the employee dining room and that she did drop forms in transit that she claims to have picked up. Update: 08/24/12:Due to full SSN and DOB being exposed, Employee A will be sent a letter offering credit protection services.

Supervisor investigating - referred to HR.
