Issue:

A VA clerk provided a VA pharmacist the last name and last 4 numbers of the SSN of a Veteran that checked-in to refill a medication. The pharmacist noticed a diagnosis that should be discussed in relationship to the medication being filled. The pharmacist began to talk with the Veteran about the side effects of the condition and the medication. The Veteran told the pharmacist he had never been told he had that condition. A VA physician was notified and discussed it with the Veteran. The Veteran realized he was checked-in under his brothers identity. The Veteran and brother have the same last names and their SSN is one digit off. It is believed the VA clerk did not properly ID the Veteran during the check-in process.

Outcome:

12/19/13: One Veteran will be sent a notification letter.
