Issue:
Based on staff interview, clinical record, and administrative document review, the hospital failed to keep Protected Health Information (PHI) confidential when Patient 1's PHI was disclosed by Employee 1 (E 1) to other employees. This failure resulted in the breach of Patient 1's PHI and possible unauthorized use. On 7/11/14 at 2:47 p.m., during an interview, The Privacy Officer (PO) stated (E 1) began asking co-workers if they knew Patient 1 (P 1) was pregnant. E 1 also showed P 1's lab results to co-workers. The PO stated E 1 should not have accessed, or revealed P 1's PHI because she had no business need to know.Patient 1's PHI breached were her laboratory results. The lab result sheet contained name, date of birth, age, and medical record number.The hospital policy and procedure titled "Use and Disclosure of Protected Health Information", dated 8/29/13, indicated, Adventist Health is committed to protecting the privacy and security of protected health information (PHI) and our commitment to the HIPAA [Health Insurance Portability and Accountability Act] Privacy Rule ..."
