Issue:

The Privacy Officer (PO) received an Outlook message from the Office of Quality, Safety and Value out of Washington D.C. regarding the need for a security report to be filed based on these facts. On 02/06/12, five sets of finger print cards were sent via UPS. They were received on 02/07/12 by VA Little Rock facility. On 03/22/12, it was discovered that four fingerprints card for four individuals are missing. The missing cards belong to subcontractors to a VHA contractor. Update: 04/02/12:Five fingerprint cards were shipped by a sub-contractor of the Office of Risk Management at VACO. All five were shipped in the same container and received and signed for at the security center in Little Rock AR. Four of the cards can no longer be accounted for. The Privacy Officer will follow up with the security center to verify they are still missing.04/05:The fingerprint cards Four employees cannot be accountedf for, therefore the four employees will be sent a letter offering credit protection services.

Outcome:

Letters are in the Director's office for signature . Appropriate actions to be taken by area involved. Letters have gone out to notify individuals see attachment. Individual that is supervisor in the area stated the information never left their building - reason only notification.
