Issue:
Today, 11/03/14, the Privacy Officer (PO) received a report that Veteran As wife received Veteran Bs medication education sheet by mail last week. Veteran A also received his own with medication. His wife provided a copy of this but did not provide original to intake staff. She wonders if her husbands materials are being misdirected. The PO or Pharmacy to call Veteran/wife. Additional investigation and notification to ensue.
Outcome:
11/03/14: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
