Issue:
On 06/19/12, a physician at one of the Community Based Outpatient Clinics (CBOC), inadvertently handed Veteran A two printed Patient Inquiry forms that belonged to two separate Veterans.The Patient Inquiry form contains demographic data, insurance information; dates of treatment, etc.. Upon realizing that he had received documentation belonging to other Veterans, Veteran A called the Call Center. The Call Center advised the Veteran to return the forms to the clinic, which he did. The clinic Manager contacted the Privacy Officer (PO) on 06/20/12 and was advised to send the forms to her so that this privacy incident could be investigated. Update: 06/26*/12:Two Veterans will be sent a letter offering credit protection services due to full name and SSN being disclosed.
Outcome:
CHIEF OF STAFF NOTIFIED OF PRIVACY BREACH SO THAT PROPER DISCIPLINARY ACTION CAN BE TAKEN. CREDIT MONITORING LETTERS MAILED ON 6/29/12.
