Issue:
Based on record review, the facility failed to safe guard Patient A's private health information (PHI). The error resulted in a breach of Patient A's PHI. Findings:During a review of the facility report dated 4/25/13, The facility reported Registered (RN 1) sent Patient A's discharge instructions to the OB Triage with another patient. The information disclosed included patient name, date of visit, Physician name, medical records number and account number. As a result of the error there was a breach in Patient A's PHIRN 1 received counseling and training.
