Issue:
A Patient reported that someone created a My HealtheVet account for him (June 2012). The Patient stated that he did not create the account. Update: 08/20/12:It was confirmed that two clerks were authenticating Veterans without obtaining their authorization. They stated that they may have misinterpreted the supervisor's guidance of "making sure patients get authenticated" as the go ahead to do the second part of enrollment. A full written report will be provided this week to management.Assocaite director and Acting Chief of COPS held a meeting with all CBOC clinical administrators along with the VISN 10 My Health-E-Vet coordinator to re-educate staff on proper procedures. Staff was given an opportunity to ask questions or raise concerns. Associate Director also had the CAC create a specific note title to scan My Health E Vet to the record (currently we are using ROI, which also includes other types of ROI disclosures). A report will be pulled so PO and others can go through and see if this is a medical center wide issue or not. Updates will follow as they become availabe.83 VACO letters printed and in director's office. Will upload redacted copy once signed.
Outcome:
VACO MHV letters mailed this morning to patients. Processes were changed and education was given to all ROI and CBOC clerks by the VISN My HealtheVet coordinator.
