Issue:

A piece of paper was found unsecured in the hallway that had a list of Veterans' names and social security numbers on it. The Privacy Officer (PO) does not have the piece of paper in his/her possession yet, however, it is in the VA police's possession. Update: 06/29/12:The list was left in the hallway because there are several moves going on in the adjacent rooms. The person thought they had thrown the list away and it didn't get it in the Shred-It container. The list was an e-mail to a home health nurse to help in taking care of those veterans because they are a high risk for being admitted to the hospital. Eight Veterans will be sent a letter offering credit protection services.
