Issue:

A Veteran initiated a complaint to the VA Police Service on 07/31/12. A Board of Investigation revealed that the alleged employee went into the Veteran's record without a need to know. Appropriate disciplinary action will be taken. Update: 09/20/12:Per the information Security Officer (ISO), the patient complained to the VA Police that the employee was harassing him. When the VA Police investigated, they found that the employee had been in the Veteran's record when the employee was not on the Veteran's treatment plan. The employee is aware that the Veteran complained to the VA Police and that the employee is being investigated. For this reason, we will offer the Veteran credit protection services instead of notification..

Outcome:

A credit monitoring letter has been sent instructing Veteran to protect himself against identity theft. Disciplinary actions are pending.
