Issue:

A VA provider handed the daily appointment list of all patients for that day, which included his, to one of her patients upon leaving his appointment. The appointment list included 7 more patients. The information of each patient included full name, full SSN, home phone number, service rating and the providers name. The patient reported this incident and returned the patient list to the Facility Information Security Officer (ISO).

Outcome:

06/24/15: The Incident Resolution Service Team has determined that 7 Veterans will be sent letters offering credit protection services.
