Issue:
A digital camera was left on a clerk's desk when a VHA employee came into office to clean. The camera was missing upon clerk's return to her desk Update: 08/25/11:The camera was used in the ENT clinic and had pictures of patients face and neck. The camera was left in a bag in a file cabinet when a VA employee left at 5:15 PM. The next morning at 8:15 AM it was discovered to be missing.08/26/11:Eleven patients will be sent a notification letter.
Outcome:
A&MMS modified policy to enhance accountability of all cameras/digital equipment issued.
