Issue:

VA Police reported a Florida Driver and Vehicle Information Database (DAVID) printout was found on the grounds at this facility by a VA employee. The printout contains a full face photo, full name, full SSN, DOB, address and motor vehicle information of a VA employee. VA Police Captain reported they immediately initiated an investigation. The Privacy Office will further investigate this matter. Update: 07/29/11:This was not the fault of VA.08/01/11:Upon further investigation this is a form used by VA Police Service, so credit protection services will be offered to the employee.

Outcome:

Employee counseled on appropriate handling of PII. Employee involved retook Privacy and Information Security training. Process changed to include documentation in the police daily journal for all NCIC/DAVID request. Credit monitoring letter mailed to affected employee.
