Issue:
A VA Physician accessed Veteran A's CPRS for information while treating Veteran B. The Veteran (Veteran B) he was treating referred to another Veteran (Veteran A) seen here at the VA and the Physician looked in Veteran A's record to confirm information.
Outcome:
06/11/14: While being examined for admission, Veteran A told his Physician that he had shared a needle with Veteran B who stated that he had Hepatitis B. The Physician accessed Veteran B's CPRS record to confirm that he did in fact have Hepatitis B. The Physician noted Veteran B's name, last 4 and diagnosis in the Veteran A's admission note. We quickly amended that note yesterday after seeing it and took out the identifying information. The note was written yesterday as an inpatient and amended yesterday. However, the Physician did in fact access the record of the other Veteran to confirm what Veteran A had relayed to her. 06/12/14: The Incident Resolution Team determined that this is an inappropriate access and that Veteran B will receive a HIPAA letter of notification.
