Issue:
Veteran A's Power of Attorney (POA) submitted documents to our Office. The documents he submitted had the wrong address. As result of the POA mistake, we sent Veteran A's notification letter and ratings decision to the wrong address. The documents that were sent contained Veteran A's SSN, name, military service dates, and listed conditions. A review of Veteran As file doesnt show that the notification letter and ratings decision were returned.
Outcome:
06/18/15: The Incident Resolution Service Team has determined that Veteran A will be sent a letter offering credit protection services.
