Issue:

The Birmingham VA Medical Center (BVAMC) Privacy Officer (PO) was provided a Report of Contact by the BVAMC Quality Manager which stated that on 05/16/13, a BVAMC RN at a clinic reported she was contacted by Patient A who stated that he did not receive medication which had been ordered for window pick up. Patient A informed the RN that upon opening a Pharmacy package received at Patient A's home, the package contained one bottle of mouthwash labeled with another BVAMC patient's name and SSN. On the afternoon of 05/16/13, the RN delivered the correct medication to the patient's home and retrieved the medication belonging to another patient and returned that medication and mailing package to the BVAMC Pharmacy.

Outcome:

06/13/13: One patient will be sent a notification letter.
